FOURTH DEGREE MEMBERSHIP APPLICATIONS:
Membership Applications (Form 4) may be obtained from any Assembly, Council, District Master or Secretary to the Master. Applications must be COMPLETED IN FULL AND SIGNED by the following:
Candidate, Proposer, Faithful Navigator, Faithful Comptroller, and the Council Financial Secretary. Failure to obtain all necessary signatures will delay the processing of the application.
Please ensure you have your 3rd Degree membership card with you on the day of the exemplification.
IMPORTANT: The original application Form 4 (all copies of Form 4) should be attached to the completed registration form provided, along with a check payable to Partick J. Glavin, Master, Indiana District and sent to:

DEADLINE FOR SUBMITTING APPLICATION/REGISTRATION FORM IS: 5 November 2024
There will be NO refunds made after this date.
EXEMPLIFICATION FEES:
Candidates – includes banquet, and lapel pin $50.00
Banquet Only – Sir Knights, Ladies, and Guests $30.00
Priests – Candidate and Guest, includes banquet $20.00
Children 12 years and younger $15.00
NOTE: Exemplification fee does not include the yearly dues payable to your assembly.
ATTIRE: **PLEASE READ - NEW UNIFORM**
Candidates must wear a Dark Suit (Black, Dark Grey or Navy Blue), plain white dress Shirt – button cuff (no French cuffs – no button-down collar shirts), conservative necktie, black socks, and plain black shoes.
Sir Knights The new uniform is to be worn as you would a business suit. Sir Knights may still wear the black tuxedo with social baldric, black bow tie and cummerbund, plain white pleated front shirt with a lay-down collar (no wing collars, frills, or color), black socks and black shoes. OLD REGALIA IS NO LONGER ALLOWED – NO EXCEPTIONS.
Priests and Deacons may wear their clerical attire. Military Candidates may wear their full-dress uniform. Police, Firefighters, and Emergency Medical Technician Candidates may wear their dress uniform.
Color Corps must wear the new uniform as designated by the Supreme Council.
Faithful Navigators, Past Faithful Navigators, Former Masters, Former Vice-Supreme Masters and Color Corps Commanders may wear the uniform of their current, past, or former office, including the jewel of their current Council or Assembly office, or Miniature Jewel(s) should be worn.
HONOR GUARD/COLOR CORPS:
Honor Guard and Color Corps will be under the direction of the District Marshal. Sir Knights possessing a Color Corps Uniform may dress for the Mass procession. Formation for Mass will be at 4:15 PM.
LADIES SOCIAL & INFORMATION: Is still being prepared.
Ladies attire for the Mass is semi-formal. Remember, your Sir Knight will be in a suit or tuxedo.
Please note that there is no hotel associated with this event, therefore all hotel reservations must be on your own.
