Reverend Fathers, State Officers, State Chairmen and Directors, Grand Knights, Program Directors, and Financial Secretaries— I hope this message finds you warm and well.
On behalf of myself and my wonderful wife, Sara, it is with tremendous honor and pride that I accept the role of Forms Chairman for the Indiana State Council of the Knights of Columbus.
My name is Steven Malone, and for those who may not know me, I would like to take a moment to share a brief overview of my background and service within the Order:
• Immediate Past Grand Knight and current Trustee of Marquette Council 3631 in Griffith, Indiana
• Past Faithful Navigator and current Pilot and Assistant Color Corps Commander for Abraham Lincoln Assembly 235 in Lake County
• Current District Deputy for District 2, covering Munster, Highland, Griffith, and Dyer, Indiana
• Member of the 4th Degree Exemplification Team for Northern Indiana
I grew up in Griffith and have been a parishioner at St. Mary’s since my teenage years. I have been a member of Marquette Council 3631 since 2010. I truly look forward to serving the State Council and all councils throughout our jurisdiction during my time in this role.
I would also like to bring an important matter to your attention. Supreme has recently updated their website, and as some of you may have already discovered, this update has caused an issue with the links from the Indiana State Council website to Supreme forms. Please be assured that the administrative team has been made aware of the issue and is actively working to resolve it as soon as possible.
In the meantime—especially with the Annual Survey of Fraternal Activity (Form #1728) due tomorrow, January 31—I have provided direct links below. These links will take you straight to the appropriate forms sections so you can access what you need without delay.

I would like to point out an error in the Council Forms section. Supreme is still listing the Semiannual Council Audit (Form #1295) as due on February 15; however, this audit is no longer required as of this year.
I expect that Supreme is already aware of the issue and working to have it corrected, but I wanted to ensure you were aware that this form is no longer required.
